Either that, or it's POSSIBLE it was plugged into a worn-out outlet; the melted prong suggests that possibility.
That's what I was thinking as far as the plug end goes, that looks like it might have been caused by poor outlet connecting making that pin a little extra warm.... I've seen that enough times.
