FSK radio - continuous wave diagnosis (SX1262)

Hello,
I'm looking for advice on where I should look to solve this issue.

I have a board with SX1262 and when I switch it to CW I get this on SDR (attachment)

I have ruled out FW issue with testing it on evaluation board where I can see nice CW as I would expect.

Somebody with a deeper understanding of these radio ICs who could give me some idea what could cause something like that?

most of all this is just not CW mode. Maybe you didn't switch it to CW mode properly or something like that.
