Flex cable repair

Hi guys.
As picture shows, I need to replace this flex cable, but I cannot find it's model. Can anyone help me?
Thank you!

Edit: I gave up finding the exact same model, because it's likely a custom one, but could anyone help me finding the jump cable + breakout board as I asked below?

There is no model, it's a custom part for particular device.

Sorry original poster, You're only going to get that from the original manufacturer. That was custom built and even if by some weird chance you find a part number it isn't an 'In-Stock' item for anyone but the O.E.M.!!! And I doubt they will sell you one anyway. Those look like user stress fractures from improper dis-assembly. Probably need a smaller crowbar.

Thank you guys.
The one need a smaller crowbar is myself. Seriously, the device is composed in such a way that it cannot be disassamled without damaging something, I damaged much more than this cable.  :horse:
So I just measured the pitch as 0.3mm, and I want a jump wire + breakout board with the same connector, then I can just solder the rest.
The plug header's structure looks the same as this: https://www.newark.com/productimages/large/en_US/77AH5017-500.jpg
I want a 13 pin one for the OP cable, and if possible, another 11 pin one.
I found cables on sold with the same plug wire shape, but wrong conductor count and/or pin pitch, not even talk about a breakout board for this small pitch...

IMHO scrape off the protective coating, and solder thin wire strands to connect broken copper traces.

It's completely tear apart now.
Also, the tiny pitch is impratical for me to hand solder.
