Phone chargers and babies electric shock

Hello everyone. Recently I came across some news about two cases of mobile phone chargers which caused death of young babies that had electric shock by putting them in mouth. There are two different cases, one in Kazakhstan and the other in India. I can't tell for sure whether these news are fake or had some changes from the original. I just wanted to understand, from a technical point of view is this possible and what could have happened? Could it be because these chargers were low cost and not certified or it could happen also with one of the famous brands?

In all reported cases like this I've seen, the issue was always in a dodgy third-party charger.

While not a good idea to stick it in your mouth, there is no real danger of electrocution from a properly designed and undamaged power supply.

Another common thing that comes up as a fire risk is that people use completely frayed cables taped over with some package tape. This is especially the case for Apple products because their cables are expensive and people want to hold off changing them.

Tragic if true, but phone chargers are more likely to be a choking hazard.

The story headlines, Toddler killed after putting a phone charger lead in her mouth while it was plugged into the wall.

Possibly an earth loop condition if the charger cable was put in a mouth and a hand touched a metal earth?

There would need to be a forensic examination of the scene, including testing of the electrical installation and a dissection of the charger by a lab with the technical skill and reputation of TUV.

The world is awash with dangerous budget and fake chargers, so this story might be possible, but not with correctly made chargers. However, it is likely the phone would have died first.

Also, "putting in mouth" could imply "lot of saliva", and that doesn't spell good on many consumer-quality live circuits.
