That last image looks like a Kycon KPPX-4P DC power plug, however as oPossum pointed out, the photo of the cable looks like a Lemo FGG.0B.304, and the connector on the pinout diagram has distinctive LEMO features. I believe the two are *not* compatible so either make some very careful shell and pin diameter measurements and match them up to the datasheets, or if you are in a hurry, buy one of each and see what fits.
Unless you can order the OEM lead from the equipment manufacturer, you'll need to make up the lead yourself - take an ordinary USB 2.0 A to B (printer) lead, hack off the B plug, beep it out in case the wire color is non-standard, and similarly confirm the ground pin position - as your pinout diagram has a list of pin numbers but no indication which is which on the connector, you'll probably need to open the equipment, identify Vbus and Gnd and work it out from there. If you get D+ and D- swapped its non-harmful but doesn't work (fails enumeration).
